Forms for Troop 34

BSA Forms can be found via the Three Fires Council Web site. 

Boys going to summer camp need Medical Exam, Class 1 and 2.  The Class 1 portion of the form needs to be refreshed annually and Class 2 portion of the form needs to be refreshed every 3 years.   The Troop destroys medical forms after being used at summer camp so please save the original of your Class 2 form for use in future years.

Adults 40 or younger can also use Medical Exam, Class 1 and 2, but those over 40 need the Medical Exam, Class 3 form.
