           a.  Class 1 & 2 (required for under 40 years old)
           b.  Class 3 (required for 40 years older)
       3. Code of Conduct (required for all)
       4. Medication Card (required for all that need medication administered)
       5. Talent Release Form (required for all)   You will only need to fill in the second half of the form (Release Authorization) if you are coming to camp early to pick up your son and the person picking him up is not listed on his medical form.
